Liposuction is a type of surgery that uses suction to remove fat from specific areas of the body, such as the stomach, hips, thighs, buttocks, arms, or neck. Liposuction also shapes these areas. That process is called contouring. Where diet and exercise alone have not been successful, liposuction can be a very effective method of body contouring. Liposuction First steps

The first step to getting liposuction in Long Island is to schedule a consultation. During this session, Dr. Nikfarjam or Dr. Micheline Goulart will explain what liposuction involves. He will assess your desired outcomes and determine whether you are the right candidate for this procedure. Your consultation session is also your opportunity to discuss any concerns or questions you have. Upon completion of the assessment, Dr. Nikfarjam will create your customized treatment plan and our Patient Coordinator will be able to help you schedule your surgery.

We will provide you with some pre-surgery instructions to follow in order to achieve the best possible results for your surgery. This will include dietary instructions and any prescription medications you will need to pick up before your procedure.

Tumescent Liposuction, typically performed on patients who need only local anesthesia, usually takes significantly longer than traditional Liposuction (sometimes as long as 4 to 5 hours). The tumescent technique gets its name from the swollen and firm or “tumescent” state of the fatty tissues when they are filled with solution.
A tumescent fluid (a mixture of saline, local anesthetic, and epinephrine) is infused into the fat to help control blood loss and fluid absorption, making the procedure very safe and well-controlled. However, because the injected fluid contains an adequate amount of anesthetic, additional anesthesia may not be necessary.

Also called Ultrasound-Assisted Lipoplasty (UAL), this technique requires the use of a special cannula that produces ultrasonic energy. As it passes through the areas of fat, the energy explodes the walls of the fat cells, liquefying the fat. The fat is then removed with the traditional liposuction technique.

UAL has been shown to improve the ease and effectiveness of Liposuction in fibrous areas of the body, such as the upper back or the enlarged male breast. It is also commonly used in secondary procedures when enhanced precision is needed. In general, UAL takes longer to perform than traditional Liposuction.

Laser-assisted liposuction uses a two-step process that involves laser probes to pierce, heat, and melt fat cells into a liquid state by focusing on the chromophores (targets of interest) which are fat and water, and then uses hollow bore cannulas to remove the fat. The lasers have a spectral region of approximately 900 to 2,800 nm which allows for a more accessible and comprehensive removal of fat. Since the fat is melted, this technique uses thinner cannulas providing minor injury to blood vessels and nerves.

Unfortunately, laser lipo does kill the fat cells, which makes them unusable for fat transfers and makes them ineligible for fat grafting procedures like a Brazilian Butt Lift or Natural Breast Augmentation.

VASER is a new and effective treatment used on the back, abdomen, neck, arms, chin, cheeks, buttocks, or legs, which targets the dissolution and removal of denser fatty tissue in these regions. Once the fatty tissue has been removed, the skin will retract, resulting in a smoother, tighter, more natural-looking appearance. With VASER® Lipo, it is now easier than ever to reduce those stubborn spots that just will not respond to diet and exercise!

VASER® Lipo, also known as LipoSelection®, is an advanced body contouring procedure that uses gentle ultrasonic energy to selectively remove unwanted body fat. An alternative to the harsh techniques of traditional liposuction, VASER® Lipo preserves important tissues like nerves, blood vessels, and connective tissue to promote smooth results and fast recovery. Power-assisted Liposuction (PAL) is an advanced liposuction technique that uses a variable-speed motor to provide reciprocating motion to the cannula which facilitates the removal of adipose tissue when combined with the reciprocating action of the surgeon’s arm. During the procedure, our surgeons utilize a cannula with a vibrating tip to break down dense deposits of fat quicker and easier for a faster recovery, which may also help make the process smoother.
There is a limit to what PAL can do, with the average amount of fat cells that can be removed being between six and eight pounds.

What to expect on the day of Liposuction surgery

Before you arrive at our practice for your procedure, Dr. Nikfarjam or Dr. Micheline Goulart will give you a list of pre-operative instructions to prepare you. These may include fasting for a certain amount of time or avoiding certain types of medication. Our surgeons will review your medical history and ask about any medical conditions you may have. Tell us about any medicines, supplements, or herbs you are taking. 

We also recommend arranging transportation to and from our practice so that you can rest after the procedure without disrupting any wound closures or sutures. 

When you arrive for your appointment, while you are being prepped for the procedure, Dr. Nikfarjam or Dr. Micheline Goulart will walk you through the process so you know what techniques he will be using, and the areas they will be focusing on, as discussed in your consultation. 

We  may mark circles and lines on the areas of your body to be treated. Photos also may be taken so that before and after images can be compared.

The first step is to administer anesthesia, to numb the area and avoid any discomfort during surgery. The type of anesthesia will depend on your comfort level as well as the amount of area being treated. 

The second step will be to make the incision on the body. Depending on which area of the body you are getting liposuction on will determine the type of technique being used. 

Then a thin hollow tube, or cannula, is inserted through the incisions to loosen excess fat using a controlled back-and-forth motion. The dislodged fat is then suctioned out of the body using a surgical vacuum or syringe attached to the cannula.

These are the techniques Dr. Nikfarjam and Dr. Goulart will choose from based on the area of your body being treated: 

  • In traditional liposuction, a cannula is inserted under the skin to break up the fat. A high-pressure vacuum is applied to the cannula to remove the fat from the body.
  • Power-assisted liposuction uses a vibrating cannula to break up stubborn fat into smaller pieces, making it easier to remove.
  • Ultrasound-assisted liposuction (UAL) uses ultrasonic vibrations to create a high amount of energy to dissolve the fat.
  • VASER liposuction is a newer generation form of UAL.
  • Laser-assisted liposuction is similar to UAL, except that it uses a laser to dissolve the fat.
  • Water-assisted liposuction uses a pressurized stream of saline to loosen fat cells.

The procedure may last up to several hours, depending on how much fat is being removed. If you’ve had general anesthesia, you’ll wake up in a recovery room. You’ll typically spend at least a few hours in the hospital or clinic so that your medical team can watch your recovery. 

Liposuction recovery

After your surgery, you can expect some pain, swelling, and bruising. Dr. Nikfarjam or Dr. Goulart may give you medicines to help control the pain and lower the risk of infection. This is an outpatient procedure, which means you will be able to return home that same day to recover more comfortably. This also means you will need to arrange a ride to and from your surgery with a family member or friend.

After the procedure, Dr. Nikfarjam may leave your incisions open or put in temporary drains to help fluid drain out of your body. Most people need to wear tight garments after surgery to help reduce swelling. This compression garment will need to be worn for a few weeks. It is important to follow the post-operative recovery instructions from Dr. Nikfarjam to ensure a smooth healing process and amazing results. 

You will most likely need to wait a few days before going back to work. It may take a few weeks before you can start your usual activities, including exercise and anything else physical or strenuous. During this time, expect some differences in shape as the remaining fat settles into position. It will take weeks to months for the swelling to go down and to see the final results.
