What is Breast Augmentation?

Breast Augmentation (Augmentation Mammoplasty) is a surgical procedure that places saline or silicone implants inside the chest to boost the size and appearance of the natural breasts. The plastic surgeon lifts breast tissue to create a pocket for the implants beneath the nipples. This procedure can also restore breast volume lost after weight reduction or pregnancy, achieve a more rounded breast shape, or improve natural breast size asymmetry. The surgery takes a few hours to complete and may result in permanent scarring, but Dr. Nikfarjam and Dr. Goulart will make every effort to make them as inconspicuous as possible.

There are many important decisions to make when you undergo breast augmentation that you must consider. We will be sure to go over all of these decisions during your consultation and discuss the pros and cons to allow you to make an informed decision. Below, are some of the important factors that you will need to consider:

  • Implant Filler: Breast implants have an exterior silicone shell, filled with either saline or a silicone gel. Saline implants tend to ripple, have the potential to leak, and can have a different feel to both the patient wearing them and the person touching them, whereas Silicone gel breast implants will ripple less, are usually softer to the touch, and tend to have a more natural feel.
  • Implant Covering: Breast implants have two types of coverings – textured and smooth. Textured implants can lead to significant rippling on the skin and can become very hard, whereas smooth implants will settle quicker and drop a little more, giving a more natural appearance of breast symmetry.
  • Implant shape: The two shapes that modern breast implants have are teardrop and round. Teardrop implants (also called anatomic) tend to rotate and have a textured covering, while round implants can eventually achieve the teardrop shape when partially placed under the pectoral muscle. Your muscle ends up pushing on the upper 2/3s of the implant, leaving the lower third unaffected by muscle bulges, eventually creating a teardrop profile.
  • Implant insertion/ incision: The location of your incision will depend on the implant type you are choosing and the size you are looking to achieve.
  • Implant Positioning: Breast Implants can be placed above or below the pectoral muscle, with some being placed partly above chest muscles AND partly below. Every patient will have different requirements, but having the implants below the chest muscle helps the silicone gel breast implants to stay softer and produce better mammogram results. Your provider will help you choose the best option for you and your body type during your consultation.
  • Cup Size: While this is often our patient’s favorite decision that has to be considered, it is also a more difficult category to discuss because, unfortunately, there’s no solidarity in the cup-size community. For example, a B could be a standard B from one brand, but for another, a B could be closer to a C. This is why we encourage our patients to focus more on the visual appearance of an implant size, and not necessarily about which letter it is associated with.

Types of Breast Enhancement

Breast enlargement surgery requires various methods, each tailored to an individual’s unique goals and body characteristics. Two of the most common methods include breast augmentation with implants or fat transfer. Your plastic surgeon will discuss the benefits of our breast procedures and help you determine which is best for you.

Saline implants are filled with a sterile saline solution, very similar to the saltwater that’s in the human body. Unlike silicone implants, saline implants can either be pre-filled or filled during the implant operation.

The silicone implant is composed of an outer silicone shell filled with silicone gel that feels like jelly.

Also known as Cohesive Gel Implants, “Gummy Bear” implants are a term used for a type of silicone breast implant known as “form-stable” or “highly cohesive” implants. These implants are made of a thicker, more cohesive silicone gel that retains its shape even if the silicone outer shell is cut or ruptured.

Fat transfer breast augmentation, also known as autologous fat transfer or natural breast augmentation, is a surgical procedure that involves using a person’s fat cells to enhance the size and shape of the breasts. In this procedure, fat is typically harvested from areas of the body where there is excess fat, such as the abdomen, thighs, or hips, through liposuction.

Smooth-walled silicone and saline breast implants are the most common choice for breast augmentations. Great for round implants, these are made of silicone gel or saline implants and have a smooth outer surface, creating a natural look and feel of natural breast tissue with a lower risk of rippling. When possible, silicone breast implants are placed beneath the chest muscle to provide better coverage, less chance of visibility, and less risk of capsular contraction.

Breast Augmentation First steps

The first step to getting a Breast Augmentation in Long Island or the New York City area is to book a consultation with Dr. Nikfarjam or Dr. Goulart. By scheduling a consultation, you will get to meet Dr. Nikfarjam, learn more about breast augmentation, and decide whether this is the right procedure for you. 

While performing their evaluation, Dr. Nikfarjam or Dr. Goulart will take everything into account, including your body type and desired aesthetic. You will also be asked questions about your medical history and lifestyle, which not only provides more insight into your life but also helps avoid any unnecessary risks. 

The next part is what most of our patients find to be the most fun, which is getting to choose the size and shape of their breast implant surgery themselves. Types of Implant Incisions

Depending on the woman’s breasts, the degree of change, and the patient’s body type, your surgeon will need to make incisions that are not only as discreet as possible but will also allow him to successfully perform breast enhancement procedures.

Each one of these incisions is designed to accommodate an implant while hiding any resulting breast augmentation scars, but keep in mind that not every incision location is available for every type of implant. Your breast augmentation specifics, such as the style and type of implant, will also help determine the best location for an incision.

The most common types of incisions for cosmetic breast surgery include:

Inframammary incision: also called a crease incision, this is one of the most common incision types. When performing breast augmentation using this technique, your surgeon will make an incision in the crease below the breast in the inframammary fold, where the breast and the chest wall meet. This approach allows for greater control and extremely precise positioning of the breast implant, however, the main drawback is that this technique does allow the scar to be more visible, especially when lying down.

Periareolar incision: the incision runs around the areola (the area around the nipple) allowing the implant to be inserted through the opening and placed into position. There are numerous benefits to this approach, including small openings, faster healing, and well-concealed or practically invisible scar tissue that blends in with the skin’s color. Unfortunately, this incision will not work for patients who have very small nipples.

Transaxillary incision: the opening is made under the armpit and is great for women who have not had prior breast enhancement, do not need a breast lift, and prefer a saline implant. After making the incision, your surgeon will place the implant into a pocket in the chest wall, and then fill the implant with saline to the prediscussed measurements. While this technique is great for hiding scarring, the armpit can no longer be used for future surgeries.

What to expect on the day of Breast Augmentation surgery

Before your surgery, Dr. Nikfarjam may give you a set of pre-operative instructions. These may include fasting for a certain amount of time and avoiding medications such as blood thinners. 

On the day of your Breast Augmentation surgery, Dr. Nikfarjam will walk you through the steps of the procedure as you are being prepped for surgery. 

To begin, a type of anesthesia will be administered, so that you do not feel any pain or discomfort throughout the procedure. Next, Dr. Nikfarjam will make the incision on the specific part of the breasts as discussed in your consultation. Incisions vary based on the type of breast implant, degree of enlargement desired, your particular anatomy, and patient-surgeon preference.

Once the incision has been made, a breast implant is inserted into a pocket either under the pectoral muscle (a submuscular placement) or directly behind the breast tissue, over the pectoral muscle (a submammary/ subglandular placement). The method for inserting and positioning breast implants depends on the type of implant, degree of enlargement desired, your body type, and Dr. Nikfarjam’s recommendations. 

After the implant is in place, the incisions are closed with layered sutures in the breast tissue and with sutures, skin adhesive, or surgical tape to close the skin.

Breast Augmentation recovery

Before leaving our care, Dr. Nikfarjam and his team will prepare you with a list of instructions to help you through the recovery and healing process to achieve the best breast augmentation results. Since this is an outpatient procedure, it is recommended that you arrange transportation to and from our practice so you can rest after your surgery. You will be given a specialized surgical bra to wear throughout your recovery, to ensure proper healing without too much movement. You can expect to experience some bruising and swelling throughout the breast area. 

Dr. Nikfarjam will advise you to rest and avoid strenuous activity for the first two weeks, after which you may be able to do some very low-impact activities. We also recommend taking some time off work or regular commitments to give yourself ample time to heal, before resuming regular daily activities.
